I hereby declare that I am legally authorized to enter into this agreement on behalf of the person/business listed above. I agree to all terms enclosed in this agreement. In addition, all processes related to the performance of services (consultation, installation, work-for-hire, etc.) provided to me by __brand__ shall remain confidential.
I am authorized to provide payment as indicated above. I understand I will be charged for the first month of service, installation fee and/or any other one-time charge on the date in which this proposal is accepted. Funds will be held in the amount listed in this proposal up to the installation date. If installation is successful, funds will be released to __brand__. If installation is not successful, funds will be returned back to me within 3 to 5 business days.
I am the renter/owner/manager of the property that service will be installed at and give permission for roof installation as needed OR I have secured such permission for roof installation as needed from the owners/managers/HOAs of the property.
Relocation of service is available for an additional fee. Contact either __sales_email__ or __support_email__ for terms and conditions.
All cancellations must be in writing and sent via your personalized portal account (portal.isp.net) or email to __billing_email__ from the email address on file with __brand__ and signed by the account holder. Cancellation requests must be submitted 30 days from the bill cycle date. If cancellation request is made prior to 30 days from the bill cycle date, you are responsible for paying the outstanding balance due. If service is cancelled prior to the end of this agreement, you are responsible for paying the balance remaining on the agreement. __brand__ will provide that balance to you upon termination of service.
All equipment installed at your premises remains the property of __brand__. You will be responsible for any personal equipment connected to __brand__’s equipment. Contact __brand__ anytime at 702.900.0000 or __sales_email__.
